Use the Adobe to fill out the form. When done, print the form and fax or (preferred) scan. Send the completed report to Bryan Eisentraut at the fax number listed on the form and a copy to Herb Haley at fax: 623-872-8143; or email to Bryan Eisentraut at the email address listed on the form with a copy to Herb Haley at:
timeclass@aol.com. The form must be sent to Bryan and Herb within 24 hours of the incident. |
